What is Premature Ejaculation?

Ejaculation is the process of getting sperm out of the body. Premature ejaculation (PE) occurs when a man or his partner ejaculates before they want to. Premature ejaculation (PE) occurs when a man ejaculates early, quickly, or before his climax. Sometimes PE isn’t a big deal. If it makes relationships worse and sex less fun, it can be annoying. Ejaculating is controlled by the central nervous system. Sexual arousal sends messages to men’s brains and spinal cords. When men reach a certain level of excitement, signals are sent from the brain to the reproductive organs. This causes the sperm to be expelled through the penis (ejaculation).

Ejaculation has two stages: Emission and Expulsion.

Stage 1: Emission

Emission is when sperm move from the testicles to the prostate and mix with seminal fluid to make semen. The vasa deferentia are tubes that help move the sperm from the testicles through the prostate to the bottom of the penis. (When you are talking about just one of these tubes, you call it a vas deferens.)

Stage 2: Expulsion

Expulsion happens when the muscles at the base of the penis contract. This forces sperm out of the penis. Usually, ejaculation and orgasm (climax) happen at the same time. Some men get to climax without ejaculating. Most erections go away after this step.

Premature Ejaculation and Erectile Dysfunction (ED)

Men who have trouble getting an erection sometimes have trouble with PE (erectile dysfunction or ED). Men with this problem can’t get or keep an erection strong enough for sex. Causes of Premature Ejaculation

What exactly causes PE is unknown, but serotonin may play a role. Chemically, your body’s cells make serotonin. In a serotonin-rich brain, ejaculation takes longer. Low amounts can make ejaculation happen faster and lead to PE.

Issues related to psychological or mental health can lead to PE, such as:

  • Guilt

Talking about the issue is a key step. Couples therapy or sex therapy can be helpful. Some exercises, like the squeeze method, may help you and your partner keep an erection going longer (read more about this in the section on treatment). Get together with your partner and learn how to relax. Performance anxiety and other forms of stress can make it difficult to do PE.

Diagnosis Premature Ejaculation

During sex with a partner and masturbation, men usually have some say over whether and when they come. If a man doesn’t feel like he can control when he ejaculates, and if he or his sexual partner(s) are concerned about it, he may be suffering from PE.

When Premature Ejaculation interferes with your sexual enjoyment, you should seek medical attention. The diagnosis is based on whether the ejaculation occurs early, late, or not at all. Most of the time, your health care practitioner will diagnose PE after performing a physical exam and consulting with you. He or she might ask the following questions:

  • How often does PE happen?
  • How long has this been going on?
  • Does this only happen with one partner, or with all of them?
  • Does PE happen every time you try to have sex?
  • What kinds of sexual acts do you do (masturbation, foreplay, sex, using body language, etc.) and how often do you do them?
  • How has PE altered your sexual behavior?
  • How are your relationships with people you know?
  • Is there anything (such as drugs or alcohol) that worsens or improves PE?

Lab tests are only done if your health care provider finds something during your physical exam.
