What is Premature Ejaculation?

Ejaculation is the process of getting the sperm out of the body. Premature ejaculation (PE) is when a man or his partner ejaculates before he or she wants to. PE is also called early ejaculation, rapid ejaculation, and premature climax. PE might not be something to worry about. It can be frustrating if it hurts relationships and makes sex less fun. If it happens often and makes things hard for you, your doctor can help.

How Does Ejaculation Work?

Ejaculating is controlled by the central nervous system. When men are sexually stimulated, signals are sent to their spinal cord and brain. When men reach a certain level of excitement, signals are sent from the brain to the reproductive organs. This causes the sperm to be expelled through the penis (ejaculation).

Ejaculation has two stages: emission and expulsion.

Stage 1: Emission

Emission is when sperm move from the testicles to the prostate and mix with seminal fluid to make semen. The vasa deferentia are tubes that help move the sperm from the testicles through the prostate to the bottom of the penis. (When you are talking about just one of these tubes, you call it a vas deferens.)

Stage 2: Expulsion

Expulsion happens when the muscles at the base of the penis contract. This forces sperm out of the penis. Usually, ejaculation and orgasm (climax) happen at the same time. Some men get to climax without ejaculating. Most erections go away after this step.

Premature Ejaculation and Erectile Dysfunction (ED)

Men who have trouble getting an erection sometimes have trouble with PE (erectile dysfunction or ED). This is when a man can’t get or keep a strong enough erection for sex. It can be hard to tell if the problem is PE or ED because an erection goes away after ejaculating. ED needs to be taken care of first. Causes of Premature Ejaculation

Even though no one knows for sure what causes PE, serotonin may play a role. Serotonin is a chemical that is made by nerves in your body. When there is a lot of serotonin in the brain, it takes longer to ejaculate. Low amounts can make ejaculation happen faster and lead to PE.

PE can have to do with psychological or mental health issues, such as:

  • Stress
  • Guilt

One important step is to talk about the problem. Counseling for couples or sex therapy can help. The most important thing for a couple to learn is how to unwind. Performance anxiety and other kinds of worry can make PE harder.

Diagnosis Premature Ejaculation

Men usually have at least some control over if and when they ejaculate during partnered sex and masturbation. PE may be present if a man doesn’t feel like he has control over when he ejaculates and if the man or his sexual partner(s) are worried about it.

When PE gets in the way of your sexual pleasure, you should see a health care provider. The diagnosis is made based on whether ejaculation happens early, late, or not at all. Most of the time, your health care provider will diagnose PE after doing a physical exam and talking with you. Some questions he or she might ask are:

  • How often does PE happen?
  • How long has this been going on?
  • Does this only happen with one partner, or with all of them?
  • Does PE happen every time you try to have sex?
  • What kind of sexual activity do you do (foreplay, masturbation, intercourse, using visual cues, etc.) and how often do you do it?
  • How has PE changed the way you act sexually?
  • How are your relationships with people you know?
  • Is there anything (like drugs, alcohol, etc.) that makes PE worse or better?

Lab tests are only done if your health care provider finds something during your physical exam.
